A Registration Form must be filled out for each individual attending so you may need more than one form. Families, mail your forms in together. Return this form with the signed Camp Lucy Rules, Waiver of Liability Form and current Rabies Certificate for each dog attending camp.

Your dog(s) will be staying with you in your cabin. You should bring a crate or small pen for your room to keep your dog safe when you are not with your dog. Camp is shared housing accommodations so there will be other dogs sharing the cabin.
